Hi,I am V.Sreeja,3rd sem dental student.This is online elog book to discuss our patient health data after taking her consent.This also reflects my patient centered online learning portfolio.  CASE SHEET :A 18 years old female came with a Chief complaint  of increase in body temperature since 2 weeks And also vomitings since 6 days  Irregular menstrual cycles once in 60 to 90 days  History of present illness The patient is asymptomatic 2 weeks ago then she is associated with continuous low grade intermittent fever associated with chills.She is also associated with 1 to 2 episodes of vomitings per day for 3 days which are non projectile, bilious,watery,greenish colour and the amount of food she had consumed get vomited and she is also associated with abdominal pain which is at umbilical region which is not radiating and the blood test report shows decreased count of platelets. Hi,I am V.Sreeja,3rd sem dental student.This is online elog book to discuss our patient health data after taking her consent.This also reflects my patient centered online learning portfolio.  CASE SHEET :A 60 years old male named Dayanandh chary presented with  Chief complaint  of abnormal involuntary movement 7 days ago,vomiting 3 days ago History of present illness The patient is asymptomatic 8 days ago.The condition started when the patient complained of abnormal involuntary movement as he consumed three litres of toddy during a family function.The next day he started feeling weak,nausea and by evening the patient experienced the first episode of seizure for about 10 minutes and during the seizures his four limbs were rigid, deviation of eye ball and frothing from mouth,  then rushed to the hospital and he also experienced 3 to 4 episodes of vomiting which are non projectile,no blood in vomit History of past illness   Tubercul

Chief complaints:Patient is asymptomatic 3 days back, shortness of breathe 3 days Pedal oedema 3days  And also decreased output of urine 3days  Dry cough of no sputum along with twitching in throat since 2 days  HISTORY WITH PRESENT ILLNESS:shortness of breathe and the patient was taken to other hospital and brought to our hospital with shortness of breathe on 12th February  HISTORY OF PAST ILLNESS: K/C/O - HTN - 2 years PERSONAL HISTORY: Diet: no proper appatite  Sleep:no proper sleep Bowl and bladder:burning sensation during urine output.
